2012 Phoenix Suns Clinic

Special Olympics Arizona and the Phoenix Suns are partnering to offer an exciting opportunity
for Special Olympic athletes. Suns alumni and other special guests will be at the event!

Participants: Open to the first 100 Special Olympics Arizona (SOAZ) athletes to register
ages 8 and up

Clinic: Wednesday, March 7th Check-in time Noon with the clinic going from 1:00pm – 3:00pm. Basketballs and water stations will be provided

Please note: No food will be provided

Attire: T-shirts will be provided and athletic attire is required (sneakers)

Waiver: In order to participate, the athletes must complete the waiver (page 2) and
have a current SOAZ Medical and Consent on file with SOAZ

Interested? Please complete the registration form (page 2 & 3) by February 27th.

You will receive a confirmation e-mail by March 2nd

In order for athletes to safely participate in any event organized by Special Olympics Arizona, the athlete must have a current Medical and Consent form on file at the State office and it is the responsibility of the HOD/coach/parent/chaperone or athlete to bring a copy to any given event.
